In this podcast, Dr. Joan Beasley, a Research Professor and the Director of the National Research Consortium on intellectual and developmental disabilities (I/DD)and Mental Health at the National Center for START Services at the University of New Hampshire, discusses the START Model. These podcasts, excerpted from an August 2022 webinar, discuss strategies for providing coordinated care for individuals with I/DD, including person-centered medical assessment considerations for individuals with I/DD, risk stratification strategies to drive interventions, and family or caregiver support, education, and outreach. Speakers describe the current gaps in care for individuals with I/DD and share strategies for delivering person-centered care for individuals with I/DD through use of interdisciplinary teams (IDTs) and innovative care coordination models.
